You’ve heard the saying “timing is everything.” Well, Dr. Burke believes this applies to timing for orthodontic treatment as well. At Burke Ortho we take timing very seriously. This means that if a patient doesn’t need braces or isn’t quite ready for braces yet, Dr. Burke will let you know that. When considering the investment in braces in terms of time lost from work, time lost from school, etc., it can be frustrating for parents when their children are in orthodontic treatment longer than they should be. Not to mention that when duration of orthodontic treatment extends beyond what it should, the teeth and gums are at greater risk for problems such as enamel decalcification, cavities, tooth root shortening, and gum diseases like gingivitis. Therefore, Dr. Burke takes pride in making sure his treatment is efficient, which means that he makes sure that his patients start treatment at the right time, taking into consideration peak jaw growth, optimal compliance and cooperation from the patient, presence of permanent teeth, etc.

Most children don’t need braces until all, or most of their permanent teeth are in. However, in some cases young children ages 7-10 may need what is called “phase I orthodontics,” which means that there is a significant orthodontic problem present that needs intervention. Because of the potential for problems present at young ages, but unknown to the parent and sometimes even the family dentist, the American Association of Orthodontists recommends that all children have their 1st orthodontic consultation by the age of 7. By this time the back teeth and some of the front teeth have erupted into place and Dr. Burke will be able to tell if phase I treatment will be needed.

The list of problems justifying phase I orthodontic treatment is short, however when done appropriately, phase I orthodontics is very important to the health of the child’s teeth and gums, as well as to ensure proper growth and development of the jaws and other dento-facial structures.

Some examples of orthodontic problems that require phase I orthodontics are: anterior crossbite (underbite of 1 or more front teeth), posterior crossbite (palate is too narrow, compared to the lower jaw), severe crowding (not enough space for erupting permanent teeth), impacted teeth (teeth stuck in the jaw and significantly out of place), severe jaw growth abnormalities, and cleft lip/palate.

This example of an 8 year old child seen by Dr. Burke in November of 2013 for consultation illustrates a partial anterior crossbite. This type of bite is very problematic because of the damage that can occur to the teeth and gums when the child bites down while chewing food. After just 6 months of phase I orthodontics, Dr. Burke was able to correct the crossbite and ensure that this patient can continue to grow and develop without damage to his teeth and gums, which would have occurred had treatment not been done.
